Understanding membership-based primary care

What it actually means, how it differs from a traditional practice, and whether it might be a good fit for you.

What is membership-based primary care?

Membership-based primary care — sometimes called direct primary care or DPC — is a model where patients pay a flat monthly fee for access to a primary care practice, rather than paying per visit or relying entirely on insurance to cover each appointment.

At Premonition Health, that fee starts at $65/month. It covers your primary care visits, better access to your care team, and communication between appointments. There are no per-visit charges within your plan.

How is this different from a traditional practice?

In a traditional primary care practice, most of the clinic's revenue comes from billing insurance per visit. This creates an incentive to see as many patients as possible, which often means shorter appointments and less time with your doctor.

Membership-based practices work differently. Because the clinic has a more predictable revenue stream, it can afford to see fewer patients per provider — which means longer appointments, same-day or next-day access, and a care team that actually knows your history.

It also means the clinic's incentives are better aligned with yours. We're focused on resolving things efficiently and keeping you healthy, not on generating more visits.

What about insurance?

This is where it gets a little nuanced — and where Premonition's model is different from pure DPC.

We offer a hybrid membership designed to work alongside your existing insurance. If you have commercial insurance, we bill it for your visits, and those payments are credited back toward your monthly membership fee. For many patients, this means their effective out-of-pocket cost is significantly less than $65/month — sometimes close to zero for months with covered visits. You also still accumulate credit toward your deductible.

We're also in-network with Medicare and Medicaid. So this isn't an either/or between insurance and membership — it's designed to work together.

If you don't have insurance, the direct membership gives you a simple, predictable cost with no billing complexity at all.

Is it right for everyone?

Membership-based primary care works especially well for:

  • People managing one or more ongoing conditions who benefit from more time and easier follow-up
  • Patients who've felt rushed or dismissed in traditional settings
  • Anyone who values being able to actually reach their doctor
  • People without insurance who want straightforward, predictable pricing

It's not a replacement for specialist care or hospital services — it's primary care, done more thoughtfully.
