Why pay a membership?
Having a monthly membership lets us care for you better
Many people ask how a monthly membership could be worth the expense. Here's how it can save you money:

Avoiding ER/UC visits: The ability to be seen quickly can save you a lot of time and money. One ER visit can cost more than the entire year of membership fees.

No unnecessary visits: We are incentivized to take care of everything in a few visits possible, saving you (and us) time and money

Cash-price labs, radiology studies and medications: Often much cheaper than through insurance, particularly if you aren't close to your deductible or have high co-pays. Check out our medication prices by clicking this link. Our labs typically cost $3-$10 each (instead of hundreds through insurance).

No surprise bills: We don't nickel and dime you, just the flat monthly fee of $65. There are no co-pays, missed/late fees or other bad surprises. For any labs, radiology studies and medications paid for cash through the clinic you will know your costs before we ever deliver care so you can decide what is worth it.

In-network with most insurance: If you have insurance we bill it as a courtesy and anything paid by your insurance is credited towards your monthly fee, lowering the cost to you and counting towards your deductible. For Medicare and Medicaid patients billing will look the same as any other primary care provider, though we offer cost-limited payment plans if needed to ensure we remain affordable.
Our Philosophy
Costs
Our priority is transparent, fair and stable prices that allow you to afford quality healthcare. We have a few different models depending on your insurance (or not having insurance):
No insurance or
commercial insurance
(most patients)
You pay $65 / month
This includes all doctor/nursing services
Labs, radiology studies and many medications are available at significantly discounted rates (in addition to the monthly fee) or we can send them through your insurance (whatever works best for you)
While monthly fees are not covered by insurance, we are in-network with most major insurance providers and will bill them for any office visits (which are covered services). Any payments from insurance will count towards your next month(s) membership fees and anything they don't pay will count towards your deductible. We don't collect co-pays, co-insurance, late/missed appointment fees or other patient responsibilities, just our monthly fee.
Medicare
We bill insurance directly
We are in-network with most major insurance providers and will bill them for any office visits as well as an advanced primary care management (APCM) fee (usually around $40-$50 a month), which is a covered service with Medicare. You will not be billed for any non-covered services (unless you choose to do weight loss medications), just for any remaining patient responsibility.
Have Medicare and Medicaid (D-SNP)? In that case all your costs are covered by insurance.
We understand that many people are on fixed incomes and can work with you to limit your costs to no more than $75 a month if Medicare leaves you with a significant patient responsibility.

Medicaid
We are currently accepting a limited number of Kansas Medicaid patients. All your costs are covered by Medicaid
Medically supervised weight loss

Notary services
Jacel is a registered notary public and is willing to provide notary services for patients and the public at $10 a document. If you require these services please call our main number and mention that you require a notary.
Meet the Team
Chad Jones
RN
Chad is an RN with more than 10 years of experience in mental health, family practice and hospital nursing. As a lifelong small-town Kansan, he is passionate about caring for people in his community and making patients feel comfortable and supported.
When he’s not at the clinic, Chad enjoys spending time with his wife and two kids. He also likes to sneak in a little adventure now and then, whether it’s snowboarding in the winter or riding dirt bikes in the warmer months.
Matt Bezzant, MD
Internist
Dr. Matt is a transplant to Kansas thanks to his awesome wife Krista (who grew up in Wichita). He received his MD at the University of Colorado School of Medicine and completed an Air Force residency in internal medicine in San Antonio. After 8 years as an award-winning active duty physician (including company-grade physician of the year for the Asia-Pacific region in 2022) he and Krista moved closer to family and he finally got to build his own practice. He is dual board-certified in internal medicine and clinical informatics and loves caring for people and designing systems to improve their care. Krista and Matt have 4 boys, 3 cats and very little free time but they love it!
Jacel Marquez
Patient Care Coordinator / MA
As a dedicated Patient Care Coordinator and Medical Assistant, Jacel is passionate about providing compassionate, high-quality care and ensuring a positive experience for every patient. With a strong background in healthcare support, she excels at coordinating care, assisting clinical teams, and maintaining a welcoming environment for patients and their families.
Outside of her role, she is a licensed Esthetician and enjoys practicing skincare and wellness in her spare time. She also values spending quality time with her 3 kids, friends, and family.
Tatumn Graham
Office Manager
Tatumn is our Office Manager, a dynamic and driven healthcare advocate with a passion for making a difference. Tatumn holds a Bachelor’s degree in Health Management & Health Sciences from Wichita State University, where she specializes in Administration and Public Health. Outside of her professional life, Tatumn enjoys spending time with family and friends, taking her dog (Maverick) on adventures, traveling, and making a positive impact wherever she goes.
Kirsten Buslinger Ornelas, PA-C
Physician Assistant
Kirsten Buslinger Ornelas is a board-certified Physician Assistant. She earned her undergraduate degree from the United States Air Force Academy and her Master of Medical Science from Wake Forest School of Medicine, where she also completed certificates in Wilderness Medicine and in Physical Activity and Nutrition.
Before beginning her career in medicine, Kirsten served as a Captain and Military Intelligence Officer in the U.S. Air Force and worked as a nationally registered EMT and lactation counselor. She is excited to continue her lifelong calling of service to others here at Premonition Health.
Outside of the clinic, Kirsten enjoys spending time outdoors with her husband and two daughters — hiking, kayaking, and horseback riding. She’s also an avid reader and CrossFit enthusiast.
