Our hybrid and direct models allow us to help you avoid unnecessary visits, get in quickly, coordinate care with specialists and act as your healthcare quarterback to make sure your healthcare team is working together to provide you the best quality care at the most reasonable cost possible.
How Premonition Health Works
Our membership fees allow us to provide:

A small patient panel (400-750 patients per doctor, compared with 1500-2000 at most practices)

Time to spend with every one of our patients (annual visits are 1 hour, any other visits can be anywhere from 5 to 60+ minutes if needed).

After hours and weekend care (generally through phone/video calls)

Same or next-day appointments all the time

Enhanced care coordination (assistance booking specialist visits and centralizing medical records)

Avoidance of junk visits (medication refills, lab result discussions, cold symptoms etc)
NOTE: Unfortunately membership fees don't count towards an insurance deductible and you cannot use HSA/FSA funds to pay them. See our example patients below to see why we think it will still save you money.
There are 2 ways to do your membership:
1) Hybrid care: You pay $50 a month for membership (which does not count towards your insurance deductible) AND your insurance will be billed for office visits and services (which counts towards your deductible and can be paid with HSA/FSA funds). We do not collect co-pays to give you additional savings. This approach allows each visit to count towards your deductible and may be more cost-effective for you.
2) Direct care: You pay a set monthly (or annual) fee that covers all services provided by the clinic (apart from labs, medications, radiology and referral services at outside clinics). You don't pay anything extra for each visit and there are no surprise bills. We do not bill insurance with this plan. If you have Medicare you must use our Hybrid plan due to legal requirements from CMS.

Why we opted for hybrid and direct primary care
Relationships
We know that excellent healthcare is built on relationships (just like most good things). Hybrid and direct care allows us to spend time to get to know you and find out what you really need. It also allows that relationship to continue around visits so that you can get help when you need it and how you need it (whether it's a phone call, text, email, video chat or visit).

We win together
Too often our modern healthcare system pits patients and doctors against each other. We structured this model so that we all win together. When you get healthier and need less care (which is our goal) you pay less for your care and we are able to help more people. The monthly payments mean that we can invest in included services like case management, ultrasound, home sleep studies, cryotherapy and in-house labs. We want you to pay less for healthcare, while increasing the value of what you get.

Insurance matters
Love it or hate it, we need health insurance. A single hospitalization, major illness or ER visit can cost thousands (if not hundreds of thousands) of dollars and devastate your finances. By working with insurance, we aim to save you money in the long run. We also work with you to tackle the complexities of the modern medical system to ensure you get the care you need in sickness or in health. Direct care offers a simpler, predictable primary healthcare cost (in addition to a catastrophic health insurance plan, which we always recommend).
